Sewage disposal - depends on the island. On Moorea, as I understand it, most individual buildings are on a septic system. Large hotels, I assume, would need their own sewerage processing plant.
If the building is adjacent or over the lagoon, you are likely to find a dual sewage system. Black water (toilets) goes to the septic system. Grey water (sinks, lavatories, showers) is piped directly into the lagoon.
Because of this design, a LOT depends on the tidal flushing of the lagoons. It works OK in Moorea, where there are multiple passes between lagoon and ocean. NOT so well on Bora Bora where there is only one pass. Bora's lagoon gets lovely algae bloom from time to time.
I read something recently about deep ocean outfalls being budgetted and/or constructed to eventually get grey water beyond the lagoons.
As to plastic litter on distant beaches, I recently saw a documentary produced by the Ocean Futures Society showing tons of litter, especially plastic litter, on uninhabited islands far out in the NW Hawaiian archipeligo. Learn about the mid-ocean gyre than brings it all there and how it is causing death in sea birds.
Made me send some money to OFS and up my game as to my personal environmental practices. Recommended viewing for all on this board.

As far as sewage, Abby Rockefeller has composting toilets in her own mansion in Massachusetts. She has an organization-RILES-that puts composting toilets in around the world. Unfortunately, because of the cleaners, cosmetics, and other stuff people pour down the drain, greywater is not that benign. I know I am on a soapbox about this topic, but it would be nice if we would quit producing toxic products that shouldn't be put down the drain.
